Strucural failure mechanisms that are normally investigated for a concrete dam during the design are: 1. Overturning, 2. Bearing Capacity of Foundation, 3. Settlement, 4. Sliding , 5. Earthquake loading, and 6.Uplift. Design details of these are discussed in most text books. Check in ' Design of Small Dams by the US Bureau of Reclamation and also in the Handbook of Hydraulics published by Mcgrawhill.
The recent publicised failure of a spillway gate at a concrete dam was the Folsom Dam gate failure in 1996(?). Discussions of this was published in Water Power and also by the USBR. Some gates have also had problems due to alkali aggregate reaction causing misalignment of the gates and grooves.
