I am looking for incident information on stilling basin floor slab and/or complete stilling basin failures. Of particular interest would be failures of a Type II or III basin with chute and baffle blocks, a drop height of 40 to 50 feet, unit discharges of 50 to 90 cfs per foot of width of spillway, and the details of the basin, floor thickness, anchorage, and under drain system. Any forensic information on the probable cause of failure and extent of failure is desired.
