1.Dam name 2.Date constructed 3.Date of failure 4.Construction method 5.Soil type 6.Cause of overtopping 7.Depth of overtopping 8.Duration of overtopping before breach initiated 9.Time for full breach to develop 10.Size of breach (length and depth) 11.Total duration of release
